Health Office
All students entering grades Pre-K or K, 1st, 3rd, 5th and all new students and any student planning to play a sport during the school year, are required to submit:
Annual Physical (Your doctor has this form) Physicals are valid for a calendar year
Immunization Record (Request from your doctor)
Students entering 6th grade need to provide proof of Tdap and 2nd Varicella immunization record before they enter school in the fall
Health History Form (Required annually in the fall for ALL students)
Dental Health Certificate (Requested but not required annually for grades)
Medications
Medications are to be brought in by a parent in the original container along with the proper forms signed by a parent/guardian and physician.
If your child has chronic headaches or other conditions needing over the counter medications or prescription medications, please provide the medication paperwork from your physician so the nurse can give your child the medication if needed.
Allergies
Students with allergies need to fill out the Allergy Action Plan with their physician and bring it to the health office by the first day of school.
Due to very severe allergies in the building, please make sure all birthday snacks AND classroom snacks are peanut and tree nut free.
Physical Education
To be excused from a physical education class, a note from a doctor is required.
Emergency Forms
Please make sure that you have entered information on your FACTS Family Portal for both emergency contacts and people who are authorized to pick up your child.
